The mission of Transitions is to help individuals, families, and Communities to break the cycles of substance abuse, family abuse, violence, crime and poverty.

We assist the indigent, the uninsured, the incarcerated, the homeless and the working poor of Kentucky.

Since 1970, Transitions has helped individuals, families and communities break the cycles of substance abuse, family abuse, violence, crime and poverty.

To carry out our mission, we provide the following to Kentucky residents:
* Residential and outpatient substance abuse treatment
* Non-medical detoxification
* Halfway house services
* Assessments
* Homeless services and housing

Almost all agency programs are located in Northern Kentucky.

Substance Abuse Treatment

Transitions offers residential treatment to Kentucky adults who cannot obtain it elsewhere. We offer a 90-day program for men and a 3-9 month program for women that allows their children ages 12 & under to accompany them. Both programs offer long-term outpatient aftercare counseling.

Our intensive outpatient program lasts 6-8 months and accepts referrals primarily from the criminal justice system. We also offer outpatient and intensive outpatient services for adolescents.

Assessments

To match youth and adults (including DUI offenders) with needed services, Transitions offers comprehensive chemical dependency assessments during business hours.
